Job description

Become the Home-Based Primary Care (HBPC) Medical Director at Our Organization

We are looking for a committed and skilled physician to take on the role of Home-Based Primary Care (HBPC) Medical Director within our Geriatric and Extended Care (GEC) Service. This position offers a unique chance to lead and enhance a crucial program dedicated to serving our nation's Veterans by delivering comprehensive, interdisciplinary care to frail and chronically ill patients in their own homes.

Position Overview

The HBPC Medical Director acts as the primary clinical leader for the HBPC program, managing care delivery across Amarillo and its associated satellite locations. This role involves a blend of leadership responsibilities (51% or more administrative) and direct patient care (up to 49% clinical), encompassing GEC services such as outpatient geriatrics, Community Living Center (CLC), and Hospice & Palliative Care.

You will work closely with the HBPC Program Director and report directly to the Chief of Geriatric and Extended Care.

Key ResponsibilitiesLeadership & Program Oversight
  • Provide strategic and clinical direction for the HBPC program.

  • Collaborate with program leadership on operations, staffing, and performance enhancement.

  • Ensure adherence to VHA, Joint Commission, and accreditation standards.

  • Drive program development and advocate for HBPC services within the VA and the broader medical community.

  • Participate in audits, reviews, and accreditation activities.

Clinical Excellence
  • Provide comprehensive primary care to medically complex Veterans in home environments.

  • Supervise and collaborate with interdisciplinary team members, including advanced practice providers.

  • Oversee patient care plans, consultations, hospital admissions, and transitions of care.

  • Conduct evaluations, diagnostics, prescribing, and ongoing management of acute and chronic conditions.

Education & Collaboration
  • Teach and mentor medical students, residents, and fellows.

  • Collaborate with VA specialists and community providers to ensure coordinated care.

  • Promote best practices and advancements in geriatric and home-based care.

Program Operations
  • Participate in team meetings to ensure effective communication among staff.

  • Oversee patient safety initiatives, chart reviews, and quality monitoring.

  • Coordinate physician coverage to maintain continuity of care.

  • Support educational efforts for caregivers and patients.

Ideal Candidate
  • Board-certified physician with expertise in geriatrics, internal medicine, or family medicine.

  • Strong leadership and team-building capabilities.

  • A genuine passion for caring for complex, aging populations.

  • Experience in interdisciplinary care and home-based services is preferred.

Additional Requirements
  • Willingness to travel locally for home visits.

  • Possession of a valid driver's license.
