As a Physical Therapist at Reliant Rehab Services LLC, you will work directly in patients' homes, creating an intimate and comfortable therapy environment. • Conduct comprehensive evaluations of patients referred by physicians, analyzing their needs and developing individualized treatment plans.
• Implement evidence-based therapeutic interventions to address a variety of orthopedic conditions, ensuring alignment with physicians' plans of care.
• Document patient progress accurately and thoroughly, utilizing electronic health records and other relevant tools to maintain compliance with healthcare regulations.
• Collaborate with multidisciplinary teams to discuss patient care, share insights, and maximize patient outcomes.
• Educate patients and their caregivers on therapeutic techniques, exercise regimens, and strategies for managing their conditions effectively.
• Meet minimum utilization goals and actively contribute to the overall success and effectiveness of our services.

In this role, you will treat a diverse patient population, including individuals recovering from orthopedic surgeries, sports injuries, and chronic orthopedic conditions. You will also have the opportunity to work with pediatric and neurologic patients, making this role multifaceted and dynamic. • Bachelor's Degree from an accredited physical therapy program recognized by the American Physical Therapy Association (APTA) or the Commission on Accreditation in Physical Therapy Education (CAPTE).
• Active State License in Physical Therapy.
• Basic Life Support Certification from the American Heart Association.
• Strong verbal and written communication skills for effective patient education and collaboration.
• Proficiency in word processing and electronic documentation systems.

• Experience in home health care settings or familiarity with home-based therapeutic interventions is a plus.
• Knowledge or specialization in orthopedic rehabilitation techniques.
• Demonstrated ability to establish strong therapeutic relationships with patients, fostering trust and engagement in their recovery process.

Our compensation package includes a competitive salary range of USD 100,000 – 120,000 per year, complemented by performance-based incentives.
