We are seeking a dedicated and skilled Physical Therapist (PT) to join our team at Theradynamics Physical Clinic, located in San Bernardino, California. The home care setting of this position allows for a unique interaction between therapists and patients, promoting a comfortable and personal environment for effective treatment. You will have the benefit of working onsite in the patients' homes, which not only provides convenience but also enables you to assess and meet their rehabilitation needs in real-time and real-world contexts. Your work schedule will be Monday to Friday, encompassing a 40-hour work week.

• Patient Evaluation and Treatment Planning: Assess each patient’s condition and functional limitations, providing a detailed evaluation to guide the development of individualized treatment plans.
• Implementation of Treatment Programs: Utilize evidence-based techniques to apply therapeutic interventions that align with the physician’s prescribed plan of care, ensuring that care is relevant and effective.
• Specialized Care: Treat a diverse range of patients, including those focused on orthopedic, pediatric, neurological, and sports medicine rehabilitation.
• Patient and Caregiver Education: Educate patients and their families about therapeutic techniques and care protocols to maximize healing and compliance.
• Client Relationship Management: Build strong, trusting relationships with patients, encouraging motivation and engagement to reach their rehabilitation goals effectively.
• Documentation: Maintain accurate records of patient evaluations, treatment plans, and progress notes, ensuring compliance with healthcare regulations.
• Performance Objectives: Meet minimum utilization goals and other business performance objectives to contribute positively to clinic operations.

In this role, you will cater to a diverse group of patients, providing care that spans various age groups and health backgrounds. Your clients will include individuals recovering from surgery, injury rehabilitation, and chronic conditions who require tailored physical therapies to regain their independence. Your empathetic approach and commitment to patient-centered care will significantly enhance the recovery journey of those you serve.

• Graduation from a physical therapy program accredited by the American Physical Therapy Association (APTA) or CAPTE.
• Bachelor's degree in Physical Therapy.
• Active State License in Physical Therapy.
• Current Basic Life Support Certification from the American Heart Association.
• Proficient verbal and written communication skills for effective interaction with patients, families, and team members.
• Strong organizational skills and proficiency in using computer systems for documentation and care management.
• A proactive approach to building rapport with patients, fostering strong therapeutic relationships to enhance treatment outcomes.

Prior experience in home health or outpatient physical therapy will be advantageous. Familiarity with assessment tools and therapeutic modalities specific to rehabilitation in various specialties will also be beneficial for candidates applying for this role.

The position offers a competitive annual salary ranging from $100,000 to $120,000, depending on experience and qualifications.
