Job description

Job Summary

Provides ongoing monitoring of key quality metrics and outcomes data. Develops and implements quarterly training for providers and staff as it relates to changes or updates to quality metrics. Provides ongoing support to providers and staff based on provider-specific data and outcomes. Utilizes Epic and other designated software to streamline reporting and benchmarking. Supports quality initiatives and develops routine communication on quality metrics and outcomes data throughout the Shannon system. Utilizes quality metric data available to identify areas of opportunity to close care gaps and develop processes for improvement.

Working Conditions

Primarily Works in a well-lighted and air-conditioned environment with period of heavy workload and stress. This role may include working in less-than-ideal home conditions, which can include exposure to extreme temperatures and environments that may not meet typical cleanliness standards such as clutter, unkept surfaces, and homes with pets. Works in various conditions.

Computer Knowledge & Electronic Equipment Use: Demonstrates ability to consistently utilize electronic equipment and online computer programs to perform job duties, including electronic documentation, and order entry.

Resource Utilization: Consistently utilizes and maintains supplies and equipment to minimize lost charges and unnecessary equipment repair-replacement.

Confidentiality: Adheres to established policies on privacy and security requirements for compliance with the Health Insurance Portability and Accountability Act (HIPAA), as applicable by Shannon Policy.

Performance: Position Specific Essential Functions

  • Responsible for developing and generating reports as well as performing business analytic services to support the decision-making process within the organization.
  • Develops, implements and conducts training programs consistent with organizational needs related to outcome measures, reporting programs, and key organizational metrics.
  • Develops strategies and plans to close identified gaps, consistently across the organization through data analysis and trends.
  • Assists in development of quality tools, analysis and display of data and writing of process improvement reports and plans; develops presentation materials in a readily comprehensible way and professional manner.
  • Facilitates personal professional development.
  • Performs other duties as assigned.
