This Home Health Physical Therapist Assistant position offers the opportunity to practice at the heart of one of metro Atlantaโ€™s most welcoming and vibrant communities. The home health setting allows you to see patients in their real-life environment, giving you the chance to make targeted recommendations about home safety, energy conservation, and equipment needs that directly impact their daily routines. You will work closely with nursing and other rehabilitation professionals to adjust care plans as patients progress, ensuring their goals align with both medical needs and personal preferences. Caseloads typically include adult and geriatric patients with a variety of diagnoses such as orthopedic conditions, post-operative recovery, neurologic impairments, cardiopulmonary issues, and chronic disease management. Most work is performed Monday through Friday, with the potential for occasional weekend or holiday visits depending on patient volume and service needs.
