Key Responsibilities
The Home Health RN is responsible for the full scope of the nursing process in the home setting.
•Personalized Patient Care: Provide high-quality, skilled nursing care to a diverse patient base, including complex and vulnerable populations, focusing on holistic health and patient-centered outcomes.
•Clinical Expertise: Perform comprehensive initial and ongoing patient assessments, administer physician-prescribed medications and treatments, and manage complex procedures such as wound care and IV therapy.
•EHR Proficiency: Maintain meticulous and timely patient documentation using our Electronic Health Record (EHR) system, ensuring compliance and seamless communication across the care team.
•Care Coordination: Collaborate effectively with physicians, physical therapists, occupational therapists, and other healthcare professionals to develop and execute individualized plans of care.
•Patient and Family Education: Educate patients and their families on disease management, proper home care procedures, and health promotion strategies to foster self-sufficiency.
Required Qualifications
To be considered for this vital role, candidates must possess the following:
•Current and unencumbered Registered Nurse (RN) license in Massachusetts .
•Minimum of one (1) year of clinical experience as a Registered Nurse, preferably in a home health, med-surg, or critical care setting.
•Demonstrated proficiency and comfort with Electronic Health Record (EHR) systems and electronic charting.
•Current CPR certification.
•A strong commitment to patient advocacy and providing compassionate care to diverse and vulnerable populations.
•Reliable transportation and a valid driver's license.
