PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Wilmington, Massachusetts.

Position Overview

We are seeking a skilled Registered Nurse (RN) to provide high-quality home health services to patients across a broad coverage area in Massachusetts. This role follows standard LHC Group home health expectations, including case management, OASIS documentation, and coordination of care across the interdisciplinary team. The ideal candidate has strong clinical skills, home health experience, and is comfortable working independently in a field-based environment.

Schedule & Work Details

  • Location: Merrimac, Haverhill, Methuen, Lawrence, North Andover, Andover, North Reading, Wilmington, Burlington, Bedford, Concord, Acton, Carlisle, Billerica, Chelmsford, Westford, Tyngsborough, Dunstable, Dracut, Lowell, Tewksbury, and surrounding areas
  • Schedule: Days
  • Weekly Hour Guarantee: 32 hours
  • On-call/Weekend Requirement: Every 3rd week
  • Pay Rate: $51.00 to $53.00 per hour
  • Mileage Reimbursement: Yes, between patient visits

Key Responsibilities

  • Manage a caseload of home health patients and provide skilled nursing visits
  • Complete OASIS assessments including admissions, recertifications, resumptions of care, and discharges
  • Develop, implement, and update individualized patient care plans
  • Perform comprehensive patient assessments and monitor changes in condition
  • Coordinate care with physicians and interdisciplinary team members
  • Educate patients and families on disease management, medications, and care plans
  • Ensure accurate, timely, and compliant documentation
  • Participate in team meetings and support overall care coordination
  • Deliver care in accordance with LHC Group standards and regulatory requirements
  • Required Qualifications
  • Active Registered Nurse (RN) license
  • Basic Life Support (BLS) certification
  • Strong clinical assessment and documentation skills
  • Ability to work independently and manage a patient caseload

Preferred Qualifications

  • Home health experience
  • Experience with HomeCare HomeBase (HCHB)
  • Strong organizational and time management skills
  • Excellent communication and interpersonal abilities
  • Ability to adapt to a fast-paced, field-based environment
