With state-of-the-art facilities, we are dedicated to delivering the exceptional, compassionate care patients need to keep moving and keep enjoying their life.

The Nurse Practitioner is responsible for providing medical care, medication management and care coordination for patients with chronic or acute pain as delegated by the supervising physician.

This position is in Podiatry and will work in the Michigan City, Chesterton, Portage offices.

Essential Job Functions:

· Obtain patient history and perform assessments via observation and examination

· Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities

· Order, perform and interpret diagnostic and therapeutic tests as needed

· Implement interventions to support the patient to regain or maintain physiological stability

· Asses and revise care plans based on physician recommendations and patient changes

· Facilitate the patient’s transition within and between health care settings, e.g., admitting, transferring, and discharging patients

· Consult with other staff regarding referrals, follow-ups and transfer of care

· Educate patients and family by providing educational tools and resources to aid in decision making

· Participate in professional development activities and attend seminars as needed

· Exercise initiative, judgement, discretion, and decision making to achieve medical objectives

· Other duties as assigned

Competencies:

· Acute and chronic care protocols; delivering physical care following treatment plans

· Assessing patient health and measure/record physiologic and growth indices

· Health promotion principles and techniques, and patient learning measurements

· Gathering and analyzing physiological, socioeconomic and emotional patient data

· Accurately evaluating patient problems in person or via phone and providing appropriate advice, intervention, or referral

· Exercising initiative, judgment, discretion, and decision-making to achieve medical objectives

· HIPAA compliance

High School Diploma or the equivalent

Preferred Education:

2 years’ Nurse Practitioner experience or orthopedic experience preferred but not required. New graduates welcome to apply

Travel:

Travel between Lakeshore Bone and Joint facilities, hospitals, and surgical centers may be required

Physical Requirements:

While performing the duties of this job, the employee may be required to sit and/or stand for prolonged periods, work longer than eight (8) hour shifts, and to work both day/evening shifts. Work may hand dexterity as well as the need to reach, climb, balance, stoop, kneel, crouch, talk, and hear. The employee must occasionally lift and/or move up to 50 lbs. While performing the responsibilities of the job, the employee is required to talk and hear. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to focus. Environmental/Working Conditions:

Work is performed in an office environment. Involves frequent personal and telephone contact with patients and with testing sites and surgery departments. Work may be stressful at times. Interaction with others is constant and interruptive. Contact involves dealing with injured sick people.
