RN Case Manager

The RN Case Manager collaborates with physicians, nurses, patients, families, and interdisciplinary teams to coordinate care, facilitate discharge planning, and promote efficient utilization of healthcare resources. This role serves as a patient advocate, helping to ensure quality outcomes, seamless transitions of care, and compliance with regulatory requirements.

Key Responsibilities

  • Assess patient needs and develop individualized discharge and transition plans.
  • Coordinate patient care across the continuum, including post-acute services and community resources.
  • Collaborate with physicians and clinical staff to identify barriers to discharge and develop solutions.
  • Perform utilization review activities and monitor appropriate resource utilization.
  • Educate patients and families regarding treatment plans, discharge instructions, and available support services.
  • Facilitate interdisciplinary care team meetings and communicate care progression.
  • Ensure documentation meets regulatory, payer, and organizational requirements.
  • Advocate for patients to ensure safe, effective, and timely care delivery.
  • Maintain knowledge of healthcare regulations, reimbursement guidelines, and case management best practices.

Qualifications

  • Active and unrestricted Registered Nurse (RN) license.
  • Graduate of an accredited nursing program.
  • Current BLS certification.
  • Minimum of 2 years of clinical nursing experience in the acute care setting required.
  • Prior case management experience required.
