Service Coordinator/Parent Educator - Full Time
Location: Lebanon OH
The WCESC/Coordinated Care Department is recruiting for a Service Coordinator/Parent Educator. This position will provide services to both youth and their parents. This position a This is a full time position. All youth served are at risk due to trauma and or have needs in two or more of the following areas: mental health, developmental disabilities, delinquency, child protective and or other issues. This position will report to the department supervisor. The Service Coordinator/Facilitator responsibilities include, but are not limited to the following:
Assist families in a strength-based team planning process
Guide team in selection of activities, interventions and services needed to address family needs and achieve team mission
Provide services in the home or community
Conduct CANS assessment
Assist teams in development of written individualized plans of care and development of crisis plans
Assist team in monitoring outcomes
Build relationships with youth, families and community members
Interface with school districts and county agencies to assist the needs of high risk youth and their families
Provide individualized parenting classes to parents of youth with behavior and or emotional issues
Candidates must have a valid state driver's license, bachelor's degree in social services or related field, experience working with children/youth with multiple complex needs, ability to track outcomes through an electronic health record system, experience in early childhood needs/development and have excellent communication skills. Ohio license as a LSW or LPC is preferred but not required.
