Job description

Job Description: Nurse Practitioner in Geriatrics and Extended Care

Our organization is seeking a Nurse Practitioner to join our Geriatrics and Extended Care Service team. This role focuses on delivering a comprehensive array of services that prioritize quality, promptness, and the mission of serving our veterans. The Nurse Practitioner will engage in advanced clinical practice, offer consultations, and manage patient care. This position allows the NP to operate fully within the scope of their education, training, and certification, as detailed in the clinical privileges associated with this role. As a licensed independent provider with advanced education and national certification, the NP will deliver evidence-based care to a diverse patient population.

Key Responsibilities
  • Develop and execute personalized plans aimed at optimizing population health and managing diseases.
  • Exhibit leadership and collaborate effectively with an interdisciplinary team.
  • Order and interpret laboratory tests, diagnostic studies, and other assessments.
  • Prescribe both pharmacologic and non-pharmacologic treatment options.
  • Initiate referrals and consultations to specialized services as necessary.
  • Educate patients and their families on health-related topics.
  • Evaluate and modify care plans as needed to ensure the best patient outcomes.
  • Embody the core values of Integrity, Commitment, Advocacy, Respect, and Excellence.
  • Provide care within the Geriatrics and Home-Based Primary Care Program, focusing on health promotion, disease prevention, geriatric care, primary care, and palliative care.
  • Deliver competent care to veterans with complex needs, utilizing interventions to enhance health capabilities and minimize unnecessary urgent care and emergency room visits, aligned with the veterans' care goals.
  • Conduct home visits, Veteran Video Connection (VVC) appointments, and telephone consultations for veterans enrolled in the Home-Based Primary Care program.
  • Perform additional duties as assigned.
Preferred Qualifications
  • Experience working with a diverse patient population, including veterans, geriatric, and high-risk patients.
  • 2-3 years of experience as a Nurse Practitioner is preferred.
