Job description

MINIMUM QUALIFICATIONS
Education - Completion of a Program (ACEND) accredited by the Accreditation Council for Nutrition and Dietetics. Master's Degree in related field preferred.
Experience - Three (3) years relevant experience in community health, clinical nutrition or public health preferred - experience working with American Indian population.
Licenses & Certification - Incumbent must be registered (RD) with the Commission on Dietetic Registration, Licensed Dietitian(LD) in Oklahoma. If incumbent has current provisional licensure, the licensure will be required within 6 months of employment. DPP Lifestyle Coach preferred, DPP Master Trainer preferred, and a current Certified Diabetes Care and Education Specialist (CDCES) is preferred.
Knowledge & Skills -
  1. Knowledge of Food Is Medicine models, including Produce Prescription and nutrition security strategies.
  2. Strong understanding of diabetes prevention, management, and health disparities, particularly among Native populations.
  3. Ability to deliver nutrition education in both clinical and community-based settings.
  4. Cultural competency and ability to incorporate traditional foods and practices into programming.
  5. Program development, implementation, and evaluation skills (data collection, reporting, outcomes tracking)
  6. Ability to build and maintain partnerships with community organizations (e.g., Elderly Nutrition Programs, food access partners)

JOB SUMMARY
Responsible for planning, implementing, and evaluating culturally relevant nutrition and health programs that integrate clinical care with community-based interventions. This position supports individuals at risk for or living with diabetes and chronic health conditions through evidence-based nutrition education, Produce Prescription programs, and community partnerships. The RD/LD serves as a key liaison between clinical providers, community programs, and FIM initiatives to improve health outcomes, increase access to nutritious foods, and promote sustainable lifestyle changes.
WORK ENVIRONMENT
Work is performed in clinical, office, and community-based settings across the Muscogee (Creek) Nation service area. Travel is required.
PHYSICAL DEMANDS
Standard clinical/community work, including travel between sites. Must hold a valid driver's license and be insurable.
ESSENTIAL FUNCTIONS
Satisfactory job performance will be determined by successful execution of the following:
  1. Lead and support Food Is Medicine (FIM) and Produce Prescription initiatives aligned with program and grant requirements.
  2. Coordinate referrals from clinical providers and track participant engagement and outcomes.
  3. Deliver chronic disease and diabetes self-management and prevention education in community and clinic settings.
  4. Provide individualized and group-based nutrition education tied to food access interventions.
  5. Develop culturally relevant curriculum incorporating traditional foods and community practices.
  6. Target high-risk populations with tailored interventions.
  7. Access community needs, assets, and barriers related to food access and nutrition.
  8. Build and maintain partnerships with local organizations, including food distribution and senior nutrition programs.
  9. Support community-driven program design and engagement.
  10. Collaborate with healthcare providers, care teams, and case managers to align nutrition interventions with patient care plans.
  11. Document services and outcomes per program and grant guidelines.
  12. Collect, track, and report program data (e.g., participation, health outcomes, food utilization).
  13. Develop measurable objectives and evaluate program effectiveness and report results within specified Program guidelines.
  14. Support grant reporting and continuous program improvement.
  15. Maintain licensure and continuing education requirements.
  16. Maintain competencies related to ADA/HIS standards and Food Is Medicine best practices.
  17. Participate in team meetings, community coalitions, and organizational initiatives.
  18. Regular attendance is required.
  19. Performs other duties as required.
