Job description

Physical Therapist (PT) – Facility-Based | Greece, NY | Part-Time

Stern at Home Therapy is hiring a Part-Time Physical Therapist (PT) to provide one-on-one therapy services at an established facility in Greece, NY.

This is a facility-based position with an established patient population, making it an excellent opportunity for a Physical Therapist who wants consistent part-time work without traveling between patients' homes.

All patients are treated at one facility, allowing you to spend more of your time providing care and less time driving between appointments.

What You'll Do
As a Physical Therapist, you'll evaluate and treat patients directly within the facility while collaborating with facility staff, physicians, caregivers, and our clinical team.

Responsibilities include:
  • Perform physical therapy evaluations and develop individualized plans of care.
  • Provide one-on-one physical therapy treatments.
  • Help patients improve strength, balance, mobility, endurance, and functional independence.
  • Provide gait training, transfer training, therapeutic exercise, and fall-prevention interventions.
  • Recommend assistive devices and adaptive equipment when appropriate.
  • Educate patients and caregivers on exercises, mobility strategies, and safety.
  • Monitor patient progress and adjust treatment plans as clinically appropriate.
  • Complete accurate and timely clinical documentation.
  • Communicate with physicians, caregivers, facility staff, and members of the interdisciplinary care team.

Compensation:
Part-Time – Minimum 10 Patients
Part-Time W-2:
  • $45–$50 per visit.
Part-Time 1099:
  • $55 per visit.

Part-Time W-2 Benefits:
