Position Title:
Patient Safety Risk Manager
Department:

Job Description:
General Description:
Under general direction, the Patient Safety Risk Manager supports systemwide patient safety and risk mitigation activities by assisting with event review, data preparation, and basic components of the patient safety program. This role helps investigate patient events, contributes to developing action plans with guidance, and supports regulatory readiness activities. The PSRM collaborates with team members and leaders to learn foundational patient safety practices and build capability in event analysis, documentation, and improvement support.
Essential Responsibilities:
Responsibilities listed in this section are core to the position. Inability to perform these responsibilities with or without an accommodation may result in disqualification from the position.
  • Assist with event review and initial assessment using established procedures.
  • Support timely completion of Root Cause Analyses (RCAs) under supervision.
  • Collect, organize, and summarize patient safety data to identify basic trends.
  • Assist in preparing action plans that address identified safety risks.
  • Participate in Environment of Care (EOC) rounding and document observations.
  • Support survey readiness activities, including document preparation and escort/guide duties.
  • Upload patient safety data into required databases with oversight.
  • Participate in after-hours call rotation for patient safety event support.
  • Attend and contribute to regular meetings (e.g., Tier 2 huddles) by preparing assigned data.
  • Identify patient safety events in alignment with policies and escalate appropriately.
  • Assist in preparing educational materials for safety campaigns (e.g., Patient Safety Week).
  • Use appropriate resources to escalate issues to leadership with guidance.
  • Participate in "Go See" activities with supervision to support follow-up assessments.

General Responsibilities:
  • Performs other duties as assigned.

Level I Minimum Qualifications:
Education: Bachelor's degree in Healthcare Administration or other related field required
Experience: At least 1 year of experience in healthcare required.
License(s)/Certification(s)/Registration(s) Required: Certified Professional in Healthcare Quality (CPHQ) required, or attainment within 12 months of hire. Current RN License required.
Level 2 Minimum Qualifications:
Education: Bachelor's degree in Healthcare Administration or other related field required
Experience: At least 3 years of experience in healthcare, clinical practice, patient safety, regulatory compliance, or risk management required.
License(s)/Certification(s)/Registration(s) Required: Certified Professional in Healthcare Risk Management (CPHRM), Certified Professional in Patient Safety (CPPS), or Certified Professional in Healthcare Quality (CPHQ) required, or attained within 12 months of hire. Current RN License required.
Level 3 Minimum Qualifications:
Education: Bachelor's degree in Healthcare Administration or other related field required
Experience: At least five (5) years of experience in healthcare, clinical practice, patient safety, regulatory compliance, or risk management required.
License(s)/Certification(s)/Registration(s) Required: Certified Professional in Healthcare Risk Management (CPHRM) required. Current RN License required.
Knowledge, Skills and Abilities:
  • Foundational understanding of patient safety and clinical risk principles.
  • Ability to collect and summarize data accurately.
  • Basic knowledge of safety reporting and regulatory concepts.
  • Strong organizational and communication skills.
  • Ability to learn RCA methodology and apply guidance from senior staff.
  • Ability to work collaboratively and escalate issues appropriately.
