Quality and Population Health Analyst

Reports To: Manager of Quality, Compliance, and Risk

Location: Caroline Clinic

OverviewThe Quality and Population Health Analyst supports Healthcare for the Homeless – Houston (HHH)'s efforts to improve quality, compliance, patient outcomes, and operational performance through data analysis, reporting, quality monitoring, and workflow optimization. Working under the direction of the Manager of Quality, Compliance, and Risk, this role assists with quality improvement initiatives, regulatory compliance activities, performance reporting, and electronic health record (EHR) optimization to support informed decision-making and organizational accountability.

The Analyst collaborates with clinical, operational, administrative, and technical teams to monitor performance, identify opportunities for improvement, support regulatory reporting requirements, and enhance documentation and reporting processes. This position plays an important role in advancing a culture of continuous improvement, data-informed decision-making, and patient-centered care.

Duties and ResponsibilitiesQuality Monitoring and Improvement

  • Conduct internal audits, chart reviews, and quality assessments to evaluate adherence to clinical, operational, documentation, and regulatory standards.
  • Identify trends, gaps, and opportunities for improvement, and support corrective action plans as well as process improvement initiatives.

Compliance and Regulatory Support

  • Assist in monitoring compliance with regulatory and accreditation standards, including HRSA, UDS, HIPAA, OSHA, CMS, and FTCA.
  • Support policy and procedure reviews, revision, implementation, and tracking activities.
  • Prepare documentation and coordinate readiness activities for audits, site visits, and regulatory assessments.
  • Participate in compliance monitoring activities and support initiatives focused on patient safety, risk reduction, operational accountability, and system reliability.

Data Governance, Analytics & Reporting

  • Develop and maintain reports, dashboards, tracking systems, and data repositories that support organizational priorities and reporting requirements.
  • Collect, validate, analyze, and interpret healthcare data related to quality, operational performance, utilization, and population health outcomes.
  • Support UDS and other regulatory reporting activities through data validation, documentation review, and workflow monitoring.
  • Assist in the development of data visualizations and reporting solutions using Excel, Power BI, Tableau, or similar tools.
  • Participate in initiatives to improve data quality, reporting processes, automation, and integration across systems.

EPIC EHR & Workflow Support

  • Support optimization of Epic workflows, templates, and reporting structures to improve documentation quality, reporting accuracy, and regulatory compliance.
  • Assist with review, coordination, and follow-up of Epic tickets, reporting requests, workflow updates, and documentation enhancement initiatives with Harris Health & IT partners.

Minimum Qualifications

  • Associate’s or bachelor’s degree in Healthcare Administration, Public Health, Health Information Management, Data Analytics, Healthcare Informatics, or a related field
  • At least 1 to 2 years of experience in healthcare quality, compliance, healthcare analytics, clinical operations, population health, or related healthcare operational functions
  • Advanced Proficiency in Microsoft Excel, including pivot tables, Power Query, formulas, filtering/sorting large datasets, data validation, and report development
  • Knowledge of HIPAA, clinical documentation standards, and healthcare compliance principles
  • Strong analytical, organizational, and problem-solving skills, with the ability to manage multiple priorities, work independently, and communicate effectively across clinical, operational, administrative, and technical teams.++

Preferred Qualifications

  • Experience in a Federally Qualified Health Center (FQHC) or other regulatory-intensive healthcare setting
  • Familiarity with HRSA, UDS, FTCA, and EHR documentation workflows (e.g., EPIC)
  • Training in QI frameworks (e.g., Plan-Do-Study-Act, Lean, or Six Sigma)
  • Experience with data visualization or business intelligence tools such as Power BI, Tableau, or similar platforms.
  • Basic familiarity with SQL, relational databases, data modeling, reporting automation, or healthcare analytics tools.
  • Experience using Epic reporting tools, including Reporting Workbench, SlicerDicer, or similar reporting environments strongly preferred.
