BCCS is a community organization that prides itself on helping members of our community recover with dignity. We enhance the quality of life by creatively providing Education, Advocacy, Prevention, Early Intervention, and Treatment Services. We promote hope and empowerment to persons and families touched by mental illness, substance abuse, HIV and multiple occurring diagnoses, and their related challenges.
What You'll Do as an LPN Nurse Navigator with Our HEP-C Program:
  • Act as a liaison between patients, providers, insurance companies and pharmacies, and assists the Nurse Care Manager with obtaining Prior Authorizations
  • Follows individuals enrolled in treatment to ensure compliance and a smooth process through treatment, provides education and guidance under the direction of the Nurse Care Manager
  • Provides education on Hep C, including risk of reinfection, possible side effects of medications, etc.
  • Administers, reads and documents hepatitis C antibody testing and HEP C RNA testing
  • Ensures individuals enrolled in the HEP C program have appointments scheduled for follow-up. Provides individuals with refills and incentives when due
  • Serve as primary point of contact for patients enrolled in RAPID HEP C treatment program
  • Coordinate all aspects of patient care, including appointments, labs, referral and follow ups
  • Venipuncture and fingerstick/phlebotomy

Knowledge, Skills, and Abilities Needed as a Nurse Navigator:
  • Clinical expertise: Understanding insurance processes and pharmacy protocols. Strong foundation in nursing practice, understanding of hepatitis C diagnosis and treatment, or willingness to learn. Ability to perform POC tests, experience in venipuncture, fingerstick and injections, IM, SQ.
  • Data and technology: Proficiency in EMR, ability to complete and create spreadsheets.
  • Communication skills and cultural competence: Skilled in cross cultural communications to support diverse patient populations with empathy and respect. Ability to provide conflict resolution.
  • Positive attitude and ability to work in teams

Requirements
Qualifications:
  • REQUIRED: LPN - Licensed Practical Nurse - in the State of Delaware
  • REQUIRED: 1-2 Years of Nursing Experience
  • PREFERRED: 2-3 Years Healthcare/Medical Experience with Substance Abuse/Addiction Medicine/Infectious Diseases
  • PREFERRED: Experience in the field of Drug and Alcohol treatment
  • REQUIRED: Valid Driver's License With 2 Points or Less
  • REQUIRED: Must Have Own Vehicle Available for Use

Schedule:
  • Monday - Friday
  • 5:30 a.m. - 1:30 p.m. or
  • 6:00 a.m. - 2:00 p.m.
  • The Nurse Navigator will support our Milford and Georgetown locations

Pay:
  • From $30/hour
  • Commensurate with experience, education, and certification / licensure!
