Job description

Position Summary
TheBehavioral Health Provider delivers comprehensive behavioral health services topatients of Health Care Partners of South Carolina. As part of an integratedhealthcare team, this position provides behavioral health assessments,individual counseling, crisis intervention, treatment planning, and carecoordination for patients across the lifespan. Working within a FederallyQualified Health Center (FQHC), the Behavioral Health Provider is committed todelivering compassionate, evidence-based, and patient-centered care whileimproving access to behavioral health services for underserved populations.
Essential Duties andResponsibilities
โ€ข Provide behavioral health assessments,diagnostic evaluations, individual counseling, crisis intervention, andtreatment planning.
โ€ข Develop, implement, and monitor individualizedtreatment plans based on patient needs and evidence-based practices.
โ€ข Coordinate care with primary care providers,behavioral health staff, nursing, pharmacy, dental, and other interdisciplinaryteam members.
โ€ข Refer patients to community resources, specialtyproviders, inpatient treatment, and other support services as clinicallyappropriate.
โ€ข Participate in integrated care initiatives thatpromote whole-person health and improve patient outcomes.
โ€ข Maintain accurate, timely, and completedocumentation within the Electronic Health Record (EHR).
โ€ข Monitor patient progress, evaluate treatmenteffectiveness, and modify treatment plans as appropriate.
โ€ข Maintain compliance with HIPAA, HRSA, FQHCstandards, organizational policies, and applicable state and federalregulations.
โ€ข Participate in quality improvement initiatives,provider meetings, continuing education, and departmental activities.
โ€ข Establish and maintain collaborativerelationships with community agencies and referral partners to improve accessto behavioral health services.
โ€ข Provide patient and family education regardingbehavioral health conditions, treatment options, coping strategies, andcommunity resources.
โ€ข Perform other duties as assigned.
Education & Experience
โ€ข Master's degree in Social Work, Counseling,Psychology, Marriage and Family Therapy, or a related behavioral health fieldrequired.
โ€ข Current South Carolina licensure in goodstanding as one of the following:
o Licensed Independent Social Worker - ClinicalPractice (LISW-CP)
o Licensed Professional Counselor (LPC)
o Licensed Marriage and Family Therapist (LMFT)
o Licensed Addiction Counselor (LAC)
โ€ข Minimum of one (1) year of experience providingbehavioral health counseling in an outpatient, community mental health, orintegrated healthcare setting preferred.
โ€ข Experience working in a Federally QualifiedHealth Center (FQHC) or community health center preferred.
โ€ข Experience using Electronic Health Record (EHR)systems preferred.
Knowledge, Skills, and Abilities
โ€ข Knowledge of behavioral health assessment,diagnosis, counseling techniques, crisis intervention, and evidence-basedtreatment practices.
โ€ข Knowledge of HIPAA regulations and the abilityto maintain the confidentiality, security, and integrity of patientinformation.
โ€ข Strong clinical judgment, critical thinking, andproblem-solving skills.
โ€ข Excellent communication and interpersonal skillswith the ability to establish therapeutic relationships with patients andfamilies.
โ€ข Ability to collaborate effectively withinterdisciplinary healthcare teams and community partners.
โ€ข Proficiency in Microsoft Office and ElectronicHealth Record (EHR) systems.
