This position offers a remote work setting, allowing you flexibility while maintaining the necessary interactions with fellow mental health professionals and patients. You will work within a climate-controlled environment that promotes daily movement and interaction with healthcare providers in our community health clinic. • Conduct comprehensive assessments to evaluate the mental health status of patients, utilizing diagnostic tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM) to formulate individualized care plans.

• Monitor patient progress through regular evaluations and provide tailored interventions that align with their mental health diagnoses and needs.

• Document all treatment records and patient interactions promptly and accurately, adhering to regulatory requirements and internal protocols.

• Collaborate with patients, their families, and other healthcare providers to develop a holistic and culturally competent treatment plan that reflects the aspirations and concerns of the individual.

• Proactively engage in enhancing patient safety and symptom management, ensuring that interventions are adjusted to achieve optimal outcomes.

• Facilitate referrals to additional services and establish working relationships with other clinicians and organizations, ensuring continuity of care and comprehensive support for patients.

• Provide mentorship and education to students, interns, and other personnel, contributing to the growth of future professionals in the field.

• Participate in clinical supervision activities, including peer reviews and audits, to foster team development and adherence to clinical standards.

Your primary focus will be on individuals and families deeply affected by cancer. This role requires a nuanced understanding of the psychological and emotional challenges that accompany a cancer diagnosis. You will work closely with patients to provide compassionate support tailored to their unique circumstances, needs, and cultural backgrounds.

• Master’s degree in Social Work from an accredited institution.

• Current State License as a Licensed Clinical Social Worker (LCSW), allowing you to practice clinically within the state.

• Experience in a healthcare environment, particularly within community health clinics or settings dealing with oncology patients.

• Familiarity with electronic health records management and ability to leverage technology to enhance patient care.

• Demonstrated ability to work collaboratively within a multidisciplinary team.
