We are seeking a compassionate, clinically skilled Primary Care Behavioral
Health (PCBH) Therapist to provide onsite behavioral health services within a fast-paced
integrated care setting. This role focuses heavily on conducting behavioral health
screenings, suicide and safety risk assessments, crisis intervention, and brief evidence based interventions in collaboration with primary care providers and interdisciplinary
teams.


The ideal candidate is experienced in integrated behavioral health, confident in assessing
acute mental health risk, and comfortable delivering brief, solution-focused care in a
medical environment.


• Patient Interaction: Participate in patient-facing activities including intake support,
psychosocial screenings, and guided patient engagement
• Care Coordination: Assist with coordinating services across medical, behavioral
health, and community-based providers to address patient needs
• Behavioral Health Integration: Support primary care teams by identifying behavioral
health needs and facilitating warm handoffs
• Clinical Support: Observe and assist in brief interventions, counseling activities, or
care management tasks
• Determine appropriate levels of care, including outpatient referral, crisis
stabilization, or emergency intervention
• Document assessments, clinical impressions, and treatment recommendations in
the electronic health record (EHR)



• Master's degree in social work, Counseling, or related behavioral health field
• Active state licensure (LMHC, LMSW, or equivalent)
• Foundational knowledge of psychosocial theory, assessment, or counseling
techniques preferred
• Strong communication, organizational, and interpersonal skills
• Spanish-speaking candidates strongly preferred

• Ability to work effectively in a fast-paced medical environment
• Excellent collaboration and interdisciplinary communication
• Ability to build rapport quickly with diverse patient populations

Site: 100% Onsite
Clinical placements are conducted at one of our Bronx Treatment Center (BTC) locations.

All sites operate during standard business hours, Monday through Friday, from 9:00 a.m. to
5:00 p.m.
