Job Overview
The Community Health Program Coordinator is responsible for identifying high-risk patients with poor health outcomes. The role is responsible for program activities such as referral coordination, eligibility checks, and monitoring CHW caseloads. The role will also support data collection and analysis for overall program evaluation. Our organization offers an innovative, evidence-based health care program that improves health outcomes for low-income, high-risk patients using Community Health Workers (CHWs). In this model, CHWs provide structured advocacy, social support, and navigation to high-risk patients to help them reach their health goals.
Minimum Requirements
  • High School Diploma required.
  • Minimum of 3 years of experience with data collection, management, and/or analysis.
  • Minimum of 2 years of experience extracting information and exporting data to other applications.
  • Experience with the Epic EMR system is a plus.

Compensation
$25.00 - $29.32 hourly
