Job description

Purpose 

The Community Health Navigator Generalist is responsible for providing excellent customer service and care navigation to support Neighborcare Health patients in meeting their care needs. They will conduct outreach to engage Neighborcare patients and help close patient care gaps with targeted outreach, education, and scheduling appointments via call or text. The Community Health Navigator Generalist will also provide basic "one-time touch" care coordination for health-related social needs to improve access to community services and support patient participation in primary care. The Community Health Navigator works in partnership and joint accountability with other Community Health Navigation team members to achieve Neighborcare’s Mission, Guiding Principles and Goals.

Compensation:  

  • The target wage range for the position is $23.25 per hour to $29.23 per hour
  • Final offers are individually based on various factors, including skill set, years of experience, location, qualifications, work schedule and other job-related reasons.

In this position you will:

  • Make outbound outreach calls to engage Neighborcare patients who have gaps in care to support access to medical, dental, and ancillary appointments, including but not limited to Apple Health Newly Assigned, Apple Health Unengaged and Members Without Office Visits (MWOV), ER Visit follow-up, Well Child Check, Chronic Condition Follow-up, Dental Recall, and Cancer Screenings.
  • Monitor patient responses to bi-directional text outreach campaigns in the mass text application as needed to respond to patients and coordinate scheduling appointments.
  • Work a report of patients identified as having one health-related social need to provide resources.
  • Provide basic, ‘one-time touch’ care coordination for health-related social needs via phone, text, and/or MyChart.
    • Examples include, but are not limited to:
      o Transportation services
      o Clothing, food and hygiene resources
      o Medical appointments and specialty referrals
      o Coordinating appropriate benefits with eligibility specialists
      o Parenting support services
      o Older adult services
      o Patient Assistance - DME supply logistics
      o Internal referrals to NCH care team members as needed
      o External Case Managers, Social Workers, or Community Health Workers as needed
  • Send patient screenings via MyChart when relevant to scheduling and outreach guidelines and assist patients in filling out screenings as needed.
  • Advocate and navigate patients both internally and in the community as needed when conducting patient outreach.
  • Maintain current knowledge of entitlement programs (e.g., DSHS, Medicaid, Social Security), community resources, and managed care patient resources; assist patients in navigating these systems to access needed services.
  • Maintain updated knowledge of community resources and organizations that provide needed services to our patients.
  • Provide cross coverage including information sharing and assistance in covering patient care duties during times of other team members’ leave
  • Perform other duties as assigned.

      Required Skills:

      • Demonstrated ability to work respectfully and effectively with individuals of diverse racial, ethnic, socio-economic, cultural, sexual orientation, and ability backgrounds.
      • Basic PC skills in MS Windows environment, 10-key and typing.
      • Ability to communicate clearly and professionally in English, both verbally and in writing.
      • Strong listening skills and ability to demonstrate tact and patience with patients and colleagues.
      • Commitment to ethical standards, data security, HIPAA compliance, and patient confidentiality.
      • Ability to follow written and verbal instructions, learn from feedback, and apply procedures using sound judgment.
      • Strong organizational skills with attention to detail, ability to multitask, prioritize, and follow through on assignments.
      • Ability to work independently and collaboratively in a team environment.
      • Ability to adapt to high levels of activity and varied conditions while maintaining reliable and timely attendance.
      • Ability to learn new processes, procedures, and software programs quickly, while demonstrating attention to detail and
        accuracy in their daily work.
      • Willingness and ability to travel to Neighborcare sites as needed.

      Preferred Skills:

      • Familiarity with medical and community health terminology, health-related social needs (HRSN), and health insurance,
        especially Medicaid.
      • Working knowledge and comfort with large data sets, electronic health records (EHRs), and practice management systems.
      • Experience working in a primary care setting, preferably within a community health or Federally Qualified Health Center
        (FQHC) environment.
      • Experience with trauma-informed care or motivational interviewing.
      • Lived experience navigating community health resources or social service systems is strongly valued.
      • Proficiency in multiple languages.

      Education/Experience Requirements:

      • High School diploma/GED or equivalent relevant experience in a health care or social service setting.
      • Must show proof of Community Health Worker Core Competency Course Certificate or obtain within the first 12 months of employment

      Preferred Requirements:  

      • Bilingual in English and any languages, including those top languages of our patient population such as Spanish, Somali, Amharic, Tigrinya, or Vietnamese.
