Job Summary

This role supports Prisma Health and is responsible for providing community support services for underserved teammates in the surrounding communities. The services will be based off needs assessments and personal referrals. This position will assist team members by sharing and locating resources for housing, food, legal services, mental health care, etc. while ensuring they're in an environment where they feel safe. 


ESSENTIAL TASKS:  


   Conduct and analyze needs assessment data to understand social determinants of health of team members and     which community resources to leverage.
   Utilize Prisma Health's LiveWell platform to connect team members in need of support and resources.
   Act as a partner and ally for team members completing applications for assistance and or benefit     enrollment.
   Liaise between team member and the emergency care fund to assist in getting financial approval for     emergency circumstances.
   Build and maintain relationships with community partners to best be able to assist team members with     needed resources.
   Communicate with team members' department managers to ensure proper support for team member.
   Conduct appropriate educational sessions on the importance of whole-person health. 
   Conduct follow up meetings with teammates 30/60/90/1 year later to ensure continued stability, and to track program successes and opportunities.
Education:      

Bachelor's degree preferred, preferably in Health Education.   MSW or Certified Health Coach Preferred.
Experience:
Two years' experience in healthcare preferred.  Ability to communicate (both verbal and written) effectively. Ability to be onsite to support all shifts (occasional weekends & nights). Ability to apply common sense understanding to carry out detailed, but uninvolved written or oral instructions. Ability to work well under time constraints.
