Job Summary
The Hospital Liaison serves as the primary point of contact between Health Care Connectors and local hospitals, healthcare facilities, and referral sources. This role focuses on building and maintaining relationships with hospital staff, coordinating patient referrals, and facilitating smooth transitions from hospital care to home health or hospice services, and visiting hospitalized patients to provide support, maintain continuity of care, and strengthen patient engagement throughout their care journey.
The Hospital Liaison works closely with clinical teams, discharge planners, and case managers to ensure patients receive timely and appropriate care after hospital discharge.
Qualifications
Required
  • Minimum 2 years of experience in healthcare marketing, hospital liaison work, or care coordination
  • Strong relationship-building and communication skills
  • Knowledge of medical terminology to engage in clinical conversation
  • Valid driver's license and reliable transportation

Preferred
  • Experience in home health, hospice, or healthcare services
  • Knowledge of hospital discharge planning processes
  • Bachelor's degree in healthcare, marketing, business, or related field (or equivalent experience)
  • Established relationships within the local healthcare community
What You'll Do
  • Develop and maintain strong relationships with hospital discharge planners, case managers, and physicians
  • Educate hospital staff about Health Care Connectors' home health and hospice services
  • Coordinate patient referrals and assist with the intake process
  • Facilitate smooth transitions from hospital to home care services
  • Communicate patient needs and referral information to the clinical team
  • Conduct hospital visits to maintain referral relationships and promote services
  • Track referral activity and assist with business development initiatives
  • Collaborate with internal teams to ensure high-quality patient care and service delivery
Compensation
Salary: $1000 per month
