Bilingual Nurse Practitioner (NP)

This Nurse Practitioner role is part of a mission-driven healthcare organization that delivers equitable, value-based care to the Medicare population, fostering wellness and prevention through team-based support and local engagement.

Salary and Benefits

  • Base Salary: $105,000 โ€“ $135,000 (commensurate with experience and region)
  • Incentive Bonus: $15,000 โ€“ $25,000 annually based on quality outcomes
  • Physician Partnership Track available
  • 6 Weeks of Paid Time Off (inclusive of vacation, holidays, and CME)
  • Annual CME Allowance: $5,000

Responsibilities

  • Full-Time Schedule: Monday to Friday, 8:00 AM โ€“ 5:00 PM
  • Patient Load: 13โ€“16 patients per day
  • Appointment Duration:
    • Routine Visit: 20 minutes
    • New/Complex Visit: 40 minutes
  • Preventive, Value-Based Care Model
  • Support Team Includes:
    • Medical Assistant
    • Scribe
    • Social Worker
  • Onsite Laboratory and Phlebotomy
  • Specialist Access via RubiconMD
  • EMRs: Canopy and Greenway
  • Participation in Community Health Events and Wellness Programs
  • Patient Transportation Within a 5-Mile Radius
  • Collaborative Setting with 3โ€“5 Providers Per Clinic

Requirements

  • Master of Science in Nursing, Doctor of Nursing Practice, or equivalent accredited NP program
  • Active Illinois NP License and DEA License
  • National Board Certification in:
    • Family Nurse Practitioner (FNP)
    • Adult-Gerontology Primary Care NP (AGPCNP)
    • Adult NP (ANP)
    • Gerontological NP (GNP)
  • Bilingual Spanish Required
  • Experience with HCC Documentation, ICD-10 Coding, and Health Risk Assessments (Preferred)
