Pay Range: $72,009.60 - $78,000 per year
Schedule: This is a full time role but the hours does varies.

What You Bring (Requirements)

  • Preferred: BSW, MSW or College Degree with at least 3 yearsโ€™ experience working with seniors in the healthcare environment. RN/LPN preferred. Certified Geriatric Care Manager preferred.
  • Experience in case management or related field

What Youโ€™ll Be Responsible For

  • Schedule and complete pre-admission assessments of prospective members which may include a mini-mental exam, depression screen, nutritional and home assessment.
  • Manage a caseload of members, including regular communication with Members and their families, assessments of needs, coordinating and scheduling services (both provided by the Plan and not provided by the Plan)
  • Initiate the Memberโ€™s Care Plan and maintain detailed records of all Member activity in the Memberโ€™s file and REPS system.
  • Activate care coordination for Members and supervise the provision of care rendered by the credentialed provider.
  • Maintain up to date files on all resources available in the community for seniors.
