Job description

Position Title: Case Manager – Renew and Empower Program
Location: 7402 N 56th Street, Suite 400, Tampa, FL 33617
Reports To: Program Manager – Renew and Empower

Position Summary

The Case Manager will provide comprehensive reentry and empowerment support to justice-impacted individuals participating in the Renew and Empower Program. The Case Manager will be responsible for intake, assessment, individualized service planning, and ongoing case management to ensure participants successfully transition into stable housing, obtain vocational training, and achieve long-term employment. This position is based out of the Tampa office and will involve regular collaboration with community partners, employers, and service providers.

Key Responsibilities

  • Conduct participant intakes, eligibility screenings, and needs assessments.
  • Develop individualized case management plans that address housing, vocational training, employment, and supportive service needs.
  • Provide consistent one-on-one case management, mentoring, and guidance to support participant success.
  • Connect participants to vocational training opportunities and track progress toward industry-recognized certifications.
  • Coordinate referrals for supportive services including housing, transportation, healthcare, benefits navigation, and counseling.
  • Maintain strong employer and community partnerships to expand second-chance hiring opportunities.
  • Document all participant interactions and progress in compliance with program and grant reporting requirements.
  • Facilitate group sessions and workshops on life skills, financial literacy, and job readiness.
  • Monitor participant progress, identify barriers, and implement problem-solving strategies to keep participants engaged.
  • Collaborate with the Program Manager and team members to meet program goals and outcomes.

Goals and Performance Expectations

  • Enroll and maintain a consistent caseload of participants as assigned.
  • Ensure at least 80% of participants complete one or more industry-recognized certifications within the program year.
  • Support at least 70% of active participants in securing stable employment or entering advanced training/apprenticeships.
  • Achieve timely completion of participant assessments, case notes, and progress documentation with 100% compliance to reporting deadlines.
  • Provide individualized barrier resolution strategies for each participant, ensuring access to housing, transportation, or benefits when needed.
  • Conduct a minimum of two employer or community partner engagements per month to strengthen participant opportunities.
  • Maintain participant retention and engagement rates in alignment with program and grant expectations.

Qualifications

  • Bachelor’s degree in Social Work, Human Services, Counseling, or related field preferred; relevant work experience may be considered in lieu of degree.
  • Minimum of 2 years’ experience in case management, workforce development, reentry services, or related human services.
  • Strong understanding of barriers faced by justice-impacted individuals and knowledge of community resources.
  • Excellent communication, documentation, and organizational skills.
  • Ability to work independently while contributing to a team-oriented environment.
  • Proficiency in Microsoft Office and database management systems.
