Job description

Registered Nurse

A Registered Nurse will provide in-home services to our patients.

Responsibilities include:

  • Providing admission, case management and follow-up skilled nursing visits for Home Health patients;
  • Administering on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager;
  • Providing hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision;
  • Revising plan in consultation with physician based on ongoing assessments and as required by policy/regulation;
  • Coordinating appropriate care, encompassing various healthcare personnel;
  • Reporting and documenting patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis;
  • Implementing patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems;
  • Coordinating/overseeing/supervising the work of Home Health Aides and providing written instructions that reflect current plan of care;
  • Discharging patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation;
  • Preparing appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes;
  • Preparing visit/shift reports, updating/summarizing patient records in a timely manner and conferring with other health care disciplines in providing optimum patient care.

Requirements include:

  • Graduate of an accredited nursing program;
  • Current licensed Registered Nurse (RN) in the state of practice;
  • Experience as an RN with one of the following preferred:
    • One year of experience in an acute-care setting within the last 24 months;
    • Six months of Home Health experience within the last 12 months.
  • Credentials in specialty areas preferred (wound care, OASIS certification, IV, ET)
  • Valid driver's license and reliable transportation;
  • Proof of automobile insurance (MVR checked);
  • Proof of current CPR certification;
  • Able to pass criminal background check and national sex offender clearance.
