Job description

Transitions LifeCare is hiring immediately. As one of the community’s industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our 8-county territory. Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.
The schedule for this position is 3 shifts per month 7:00pm-7:30am.

Responsibilities:
  • Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patients
  • Work collaboratively with the IDG Team and other internal/external partners in achieving patient care goals
  • Use professional nursing judgement to individualize the plan of care based on assessment of the unique needs of each patient.
  • Maintain comfort, privacy, and dignity of our patients
  • Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance

Requirements:
  • Registered Nurse with current NC license; BSN preferred.
  • Must have reliable transportation to travel to, enter and function in the homes/facility of any patient/family in the service area to assess and provide patient care.
  • Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
