Connected Home Health is seeking experienced & caring Home Health Registered Nurse (RN) to join our amazing team! Would enjoy a flexible work schedule to achieve that work-life balance? Are you a Registered Nurse that is committed to providing LIFE CHANGING SERVICE to patients in the patient's home? Our Registered Nurses are passionate about helping patients pursue their health journey. Connected serves patients in the greater Portland, OR area. The ideal candidate is a dedicated patient advocate who is home health savvy, compassionate, experience in working with interdisciplinary team, driven, flexible, creative, enthusiastic, technologically adept, fun (!), quality focused, positive, and overall awesome.

Essential Job Functions/Responsibilities
• Provides direct patient care as defined in Oregon's Nurse Practice Act
• Implements plan of care
• Provides accurate and timely documentation consistent with the plan of care
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse
Qualifications
• Graduate of an accredited practical nurse or vocational nursing program
• One year nursing experience preferred
• Currently licensed as a licensed registered nurse in the state
• Complies with accepted professional standards and practice
• Demonstrates good verbal and written communication, and organization skills
• Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order
• Possesses and maintains current CPR/BLS certification from AHA
