Position Summary
The Licensed Practical Nurse (LPN) or Registered Nurse (RN) provides nursing care and support to residents within Hacienda HealthCare's Group Homes and Intermediate Care Facility. This position is responsible for promoting resident health, safety, independence, and quality of life while maintaining compliance with Arizona State regulations, the Nurse Practice Act, and organizational policies.This is a safety-sensitive position.
Essential Responsibilities
Resident Care
  • Provide for the safety, emotional, and physical needs of residents
  • Ensure residents are clean, well-groomed, and appropriately dressed
  • Assist residents with hygiene, bathing, meals, and activities of daily living as needed
  • Position and reposition residents as required
  • Promote emotional and physical stimulation appropriate to each resident's needs
  • Support resident independence and quality of life

Clinical Nursing Responsibilities
  • Administer medications and prescribed treatments
  • Monitor and assess resident health status
  • Obtain and document vital signs
  • Collect specimens as ordered
  • Administer enteral nutrition and feeding support as applicable
  • Reorder medications as needed or notify appropriate personnel
  • Report changes in resident condition to appropriate clinical leadership
  • Safeguard and maintain medical equipment and supplies

Documentation
  • Complete Nursing Notes and required clinical documentation
  • Maintain Medication Administration Records (MARs)
  • Complete Incident Reports
  • Complete Change of Condition Reports
  • Complete Seizure Reports as required
  • Ensure all required documentation is completed prior to the end of the shift

Intermediate Care Facility (ICF) Duties
  • Participate in Active Treatment programs
  • Collaborate with Respiratory Therapists to ensure quality delivery of care
  • Review respiratory assessments and intervene as needed
  • Monitor the quality of care provided by non-licensed staff
  • Review and approve documentation completed by non-licensed staff
  • Participate in ISP meetings as requested
  • Maintain working knowledge of ICF-ID regulations
  • Provide instruction, guidance, and assistance to healthcare team members and family members
  • Provide first aid, TB testing, and vaccinations as requested

Group Home Duties
  • Transport residents to appointments and community activities as needed
  • Perform routine vehicle safety inspections
  • Maintain required transportation documentation
  • Maintain a valid driver's license and acceptable driving record
  • Supervise Hab Techs regarding resident care when applicable

Compliance & Training
  • Maintain compliance with all applicable company policies and procedures
  • Attend required training sessions
  • Follow PPE and safety requirements
  • Maintain required certifications and credentials
  • Comply with Article 9, Prevention and Support, CPR, TB Testing, Fingerprint Clearance Card, and other position requirements as applicable

  • Required: Current Arizona Licensed Practical Nurse (LPN) or Registered Nurse (RN) License in good standing
  • Current CPR Certification
  • Graduate of an accredited nursing program
  • Strong communication, assessment, and documentation skills
  • Group Home Positions: Valid Arizona Driver's License; ability to meet company driving requirements; ability to work at Phoenix-area Group Home locations; two (2) years of nursing experience preferred
  • ICF Positions: Ability to obtain and maintain ACLS certification within six (6) months of hire; ability to obtain and maintain PALS certification within six (6) months of hire; experience working with individuals with developmental disabilities preferred but not required
