We are currently seeking a Registered Nurse (RN) to join our Home Health team. This role offers the opportunity to build meaningful relationships with patients, work independently, and make a tremendous impact in the community—while receiving competitive pay and excellent benefits.

JOB SUMMARY

The Home Health Registered Nurse (RN) plans, organizes, and directs home health services while providing high‑quality, patient‑centered care. The RN collaborates with an interdisciplinary team to develop individualized care plans and deliver skilled nursing services in the patient’s home.

This position is ideal for nurses who value autonomy, patient interaction, and the ability to make a direct impact.

RESPONSIBILITIES

  • Develop, implement, and revise individualized plans of care with patients, caregivers, and providers
  • Provide skilled nursing care, including preventive and rehabilitative services per physician orders
  • Deliver patient and caregiver education to promote independence and safety
  • Offer counseling and support to patients and families
  • Maintain accurate and timely clinical notes and documentation
  • Communicate regularly with physicians and healthcare team members regarding patient status
  • Identify and coordinate discharge planning needs
  • Ensure care is delivered in compliance with all regulatory and company standards

QUALIFICATIONS

  • Active California RN license (required)
  • At least 2 years of nursing experience, preferably in home health or public health
  • Current CPR certification
  • Valid driver’s license and insured, reliable vehicle
  • Strong clinical judgment, communication, and documentation skills
  • Ability to work independently and manage time effectively
  • Able to lift, transfer, and position patients as needed
  • Comfortable with walking, standing, bending, and other physical activity required in home care
