Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting personalized treatment plans to meet their needs
• Deliver physical therapy interventions that enhance mobility, strength, balance, and overall function
• Engage in collaborative partnerships with nurses, physicians, case managers, and rehabilitation specialists to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge processes, and home exercise programs
• Maintain accurate documentation of evaluations, treatments, and progress in line with facility and regulatory standards
• Participate actively in care team discussions and interdisciplinary meetings to ensure cohesive care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (neurological, geriatric, etc.) are advantageous, but not essential

Skills

• Prior experience in related settings is strongly preferred
