Job description

About Us

At Greenbrook Medical, we believe seniors deserve more from the healthcare system-more time, more care, more coordination, and more heart. We provide high-touch, relationship-based primary care to seniors, built around one simple idea: deliver the kind of care we'd want for our own parents.

Founded by two brothers inspired by their father's pioneering work in Medicare Advantage, Greenbrook is deeply personal and proudly modern. We quarterback our patients through their healthcare journey, making sure they're never alone in a complex system. Our business model is designed around patient outcomes, not volume-so we only succeed when our patients thrive.

With roots in Tampa Bay and a partnership with Tampa General Hospital, we're growing thoughtfully to bring our model to more communities. Our team is the heart of it all: mission-driven, values-oriented, and relentlessly committed to taking the best care of our patients.

About the Role

We are seeking a detail-oriented and organized Referral Coordinator to join our team. The Referral Coordinator will be responsible for managing the referral process from start to finish, ensuring timely communication between patients, providers, and insurance companies. This role will also support the clinical team by fostering meaningful relationships with patients and their families and caring for our patients in a hands-on and personalized way. Reporting to the Center Manager, this role offers the opportunity to take on more responsibility as we grow.

  • Partner with providers and other staff to create seamless patient experiences.
  • Contribute to care team discussions to enhance patient outcomes.
  • Maintain a patient-centered mindset while upholding clinic standards of care.
  • Be an extension of our MA team

About You

  • Experience:ย

    • 2+ years handling referrals from primary care to specialist offices.
  • Certification: Medical Assistant Certification is preferred.
  • Skills:
    • Strong communication and interpersonal skills to build trust with patients and team members.
    • Ability to multitask, work independently, prioritize, and work effectively in a fast-paced environment.
