Human Touch Home Health is seeking compassionate and dependable Registered Nurses (RNs) for PRN home health opportunities throughout Northampton County, Pennsylvania.
Patient opportunities may include Bethlehem, Easton, Nazareth, Hellertown, Pen Argyl, Wind Gap, Bangor, Mount Bethel, Roseto, East Bangor, and surrounding communities.
You don't have to cover the entire county. Build your caseload around your availability and the areas that work best for you.

What You'll Do
  • Complete skilled nursing visits in patients' homes
  • Perform comprehensive nursing assessments
  • Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
  • Develop and update individualized plans of care
  • Provide wound care, medication management, and disease education
  • Monitor changes in patient condition and communicate concerns promptly
  • Educate patients and caregivers
  • Coordinate care with physicians, therapists, and other members of the care team
  • Complete accurate and timely documentation using WellSky

Qualifications
  • Active Pennsylvania Registered Nurse (RN) License Required
  • Graduate of an accredited nursing program
  • Current CPR/BLS Certification
  • Valid Driver's License and reliable transportation
  • Strong clinical assessment and communication skills
  • Ability to work independently in patients' homes
  • Home Health and OASIS Experience Preferred, but Not Required
  • WellSky Experience is a Plus
