Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long-term care, or assisted living.
PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff.

Responsibilities

You Can Expect:

  • Monday – Friday 8:00am – 5:00pm – no weekends or holidays!
  • Dedicated team of providers, support staff, physical therapists, occupational therapists, dieticians, and social workers
  • 5 patients per day – small volume
  • Call is once every 7 weeks, 5pm – 8am, you are not expected to go out, calls are triaged before reaching you!
  • This opportunity is perfect for someone looking for meaningful relationships with our elderly population.
     
Qualifications
  • Required Education: Masters Degree from an accredited Physician Assistant School
  • 2 years of Physician Assistant experience 
  • Required Certifications and Licensures: Current licensure by the State of Virginia as a Physician Assistant ACLS (required only for LFM)
  • Experienced providers in palliative, hospice, primary care, or long-term care are encouraged to apply
