About the Role:
Odyssey Hospice Care LLC, DBA Touch of Grace Hospice, is seeking a compassionate and dedicated Part-Time Licensed Medical Social Worker to join our Hospice team in the Rochester Hills and Kimball Twp., MI areas. This is a meaningful opportunity to make a profound difference in the lives of patients and families during life's most sensitive moments. Responsibilities:
  • Complete initial and ongoing psychosocial assessments within required timeframes.
  • Develop and update individualized social work plans of care.
  • Provide counseling, emotional support, crisis intervention, and end-of-life education.
  • Assist with advance directives, funeral planning, caregiver stress, family conflicts, and placement concerns.
  • Assess patient safety, caregiver availability, home conditions, and possible abuse, neglect, or exploitation.
  • Participate in interdisciplinary group meetings and contribute to patient care planning.
  • Coordinate services with nurses, physicians, hospice aides, chaplains, facilities, and community agencies.
  • Provide services in private homes, assisted-living communities, nursing facilities, and hospitals.
  • Assist with discharge planning, facility placement, and continuity of care.
  • Refer patients and families for bereavement support when appropriate.
  • Complete accurate and timely documentation in the electronic medical record.
  • Follow Medicare hospice requirements, Michigan regulations, HIPAA, and Odyssey Hospice policies.
  • Attend required meetings, training, competency evaluations, and quality-improvement activities.
  • Travel throughout the Kimball Township and Rochester Hills service areas.
Requirements:
  • Master of Social Work degree from a Council on Social Work Education-accredited program.
  • Current, unrestricted Michigan Licensed Master Social Worker (LMSW) license.
  • Clinical designation or applicable clinical experience preferred.
  • At least one year of social work experience in a healthcare setting.
  • Hospice, palliative-care, home-health, geriatric, or long-term-care experience preferred.
  • Knowledge of end-of-life care, grief, family dynamics, advance-care planning, and community resources.
  • Strong counseling, communication, organization, and documentation skills.
  • Ability to work independently and collaborate with the hospice interdisciplinary team.
  • Valid driver’s license, reliable transportation, and current automobile insurance.
  • Ability to travel between Kimball Township, Rochester Hills, and surrounding patient locations.
  • Ability to complete required background checks and pre-employment requirements.
