Nurse Practitioner Or Physician Assistant Opportunity

Holyoke Medical Center is a trusted, independent community hospital that has cared for the Pioneer Valley for over 125 years. We strive to deliver high-quality, patient-centered care close to home. We’re excited to welcome an experienced Nurse Practitioner or Physician Assistant to join HMC Kidney Associates, a well-established and growing nephrology practice. This is a great opportunity to step into a supportive environment where you can build meaningful patient relationships and make a real impact.

In this role, you’ll work closely with physicians and care teams to provide high-quality care to patients with kidney-related conditions. Your responsibilities will include:

  • Performing patient evaluations, including pre-operative histories and physicals
  • Educating patients on procedures, treatment plans, and ongoing care
  • Managing and monitoring treatment plans within your scope of practice
  • Prescribing medications and therapies in collaboration with physicians
  • Supporting preventive care, health maintenance, and patient counseling
  • Maintaining accurate and timely documentation
  • Collaborating with the care team to ensure smooth patient flow and coordination
  • Participating in ongoing training, education, and quality improvement efforts

What we’re looking for:

  • Nurse Practitioner or Physician Assistant with current Massachusetts licensure
  • NCCPA certification (for PAs)
  • At least 3 years of clinical experience
  • Nephrology experience preferred
  • Strong communication, clinical judgment, and teamwork skills
  • Comfort working with adult and geriatric patients in a diverse community
