Partners in Care Counseling Center in San Jose, California, is seeking a dedicated and fully licensed mental health counselor to join our specialized team focusing on geriatric mental health. This is a full-time remote position that allows flexibility while providing essential care to our elderly patients. You will be instrumental in helping improve the mental wellbeing of older adults facing various psychiatric disorders.

This role offers a supportive and collaborative remote work environment. While you will primarily work from home, you will remain connected with a dynamic team dedicated to geriatric mental health through regular virtual meetings and case consultations. The standard work schedule is Monday to Friday, from 9:00 AM to 5:00 PM, with occasional evening shifts depending on patient needs.

• Conduct thorough mental health assessments for older adults, understanding their unique emotional and psychological needs.
• Develop and customize treatment plans that reflect each patient’s specific circumstances, including their cognitive and physical health considerations.
• Provide a range of therapeutic interventions including individual and family counseling sessions focused on addressing geriatric psychiatric conditions.
• Collaborate closely with geriatric psychiatrists and other healthcare professionals to deliver holistic, integrated patient care.
• Offer immediate support and crisis intervention during mental health emergencies involving elderly patients.
• Educate patients and their families about mental health issues, treatment modalities, and resources available in the community.
• Maintain precise and confidential patient records in compliance with legal and ethical standards.
• Participate actively in team meetings to enhance clinical knowledge and treatment strategies.
• Stay updated on current research and best practices in geriatric psychiatry to continually improve patient outcomes.

The primary focus of this position is the geriatric population, comprising older adults who often face complex mental health challenges. You will engage with individuals dealing with depression, anxiety, Alzheimer’s disease, and other age-related mental health issues. Your role is crucial in ensuring that these patients receive empathetic, informed, and effective care tailored to their unique circumstances.

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LCSW, LPCC, or LMFT in California.
• Strong understanding of geriatric mental health trends and challenges.

While not mandatory, preference will be given to candidates with experience in working with the elderly population, particularly those with dementia or other cognitive impairments. Familiarity with evidence-based treatment methodologies and previous work in collaborative healthcare settings will enhance your application.
