Why This Role Is Unique
Geriatric Care Management offers a level of flexibility and autonomy that is rare in traditional healthcare roles. Care managers structure their day based on client needs rather than a traditional 9-5 schedule. The work includes a mix of in-home visits, care coordination, family communication, and advocacy, allowing for a dynamic and meaningful workday.
Travel is a regular part of this role. Care managers spend a significant amount of time in the field visiting clients, attending appointments, and coordinating care across Northern New Jersey.
This role is ideal for a clinician who enjoys working independently, being out in the community, and building long-term relationships with older adults and their families.
