Position Description:

Title: Case Manager (Temp) Reports To: Project Manager

Hours: Full Time (100% FTE) Supervises: None

FLSA Status: Non-Exempt (Hourly) OSHA Exposure Category: II

Position Summary:

The person in this position will be responsible for linkages to community resources to meet the goals and needs of clients in the program. The case manager will assess the strengths and challenges of clients using a person-center approach, facilitating a plan of care for each individual. Essential Job Functions include those listed below.

Essential Job Functions:

  • Case management of a large caseload of participants, up to 30 at a time;
  • Have a strong working knowledge of supportive services, and community resources;
  • Ability to link clients to needed programs and provide warm handoffs;
  • Heavy documentation;
  • Assist clients in securing documentation and services needed to provide proper services;
  • Implement and enforce an Individual Service Plan approach (ISP) using positive messages, home visits, communication and problem-solving skills.
  • Provide referral services for clients and family to appropriate services and community resources for outreach, education and community services;
  • Attend required monthly Guam Homeless Coalition Continuum of Care meetings and/or trainings;
  • Serve as an advocate/liaison for youth and families at court, school, and Wellness Recovery Action Plan (Wrap) Team meetings and with other collateral agencies regarding programmatic issues;
  • Conduct all necessary administrative functions including ensuring client completion, maintenance and submission. Ensure availability of documents for files and report all related issues to the Case Manager Supervisor;
  • Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and
  • Perform all other duties as assigned.

Essential Qualifications:

Education:

Graduation from an accredited four-year college with a degree in Social Work, Psychology, Public Health, or a related field.

Experience and Competencies:

  • Two (2) years of experience in providing case management is preferred;
  • Intake and assessment experience is preferred;
  • Marketing and/or sales experience preferred;
  • Bilingual is preferred;
  • Excellent communication skills;
  • Ability to exercise good judgment and discretion;
  • Ability to work well in a team environment;
  • Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality;
  • Professional appearance and demeanor;
  • Must be culturally/linguistically sensitive to populations served;
  • Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance is required; and
  • CPR Certification and First Aid Certification.
