CCSNH Rural Healthcare Transformation North West Central High School Career Coach & Navigator
General Summary: Under the RHT Regional Lead, the RHT High School Career Coach & Navigator supports the Rural Healthcare Transformation (RHT) grant by guiding high school students through healthcare career exploration, preparation, and transition planning into postsecondary education, training programs, and healthcare employment pathways. This role focuses on building early awareness, removing barriers, and ensuring students are connected to healthcare career pathways aligned with rural workforce needs.
Principal Duties and Responsibilities:
  • Provides career coaching and navigation services to high school students interested in rural healthcare careers.
  • Supports students in exploring healthcare pathways including nursing, behavioral health, allied health, and long-term care careers.
  • Conducts outreach and career exploration activities in high schools, CTE programs, and community settings.
  • Helps students understand and access healthcare education pathways, including dual enrollment, pre-apprenticeship, and post-secondary training programs.
  • Identifies and addresses barriers to student participation, including transportation, academic readiness, financial limitations, and access to support services.
  • Connects students and families to workforce and education resources, including scholarships, training programs, and support services.
  • Works closely with educators, counselors, and RHT staff to support student transition planning from high school to healthcare careers.
  • Facilitates career readiness activities such as job shadowing, healthcare exposure visits, and career planning sessions.
  • Promotes equitable access to healthcare careers for underrepresented and rural student populations.
  • Maintains accurate documentation, case notes, and reporting related to student engagement and outcomes.
  • Collaborates with employers, colleges, and workforce partners to strengthen rural healthcare talent pipelines.
  • Participates in school-based and community-based events to promote awareness of healthcare careers and RHT opportunities.
  • Complies with all CCSNH, state, and federal rules and regulations.
  • Performs other duties as assigned.

Qualifications:
Education: Bachelor's degree in human services, education, social sciences, healthcare, public administration, or related field.
Experience: Two(2) years of experience in career counseling, case management, workforce development, healthcare support services, or community-based human services. OR
Education: Associate's degree in human services, education, social sciences, healthcare, public administration, or related field.
Experience: Four(4) years of experience in career counseling, case management, workforce development, healthcare support services, or community-based human services.
License/Certifications: Valid Driver's License or access to statewide transportation.
Additional Desired Qualifications:
  • Experience working with high school students or youth development programs
  • Knowledge of healthcare career pathways and education systems
  • Experience working with underserved or rural student populations
  • Familiarity with career coaching, advising, or case management
  • Experience with workforce or education grant programs
  • Knowledge of support services and funding systems
