Work Environment

The role is an onsite position located in our specialized Rehabilitation Facility in Leesburg, Virginia, where we provide services to a diverse clientele. You will be working alongside a multidisciplinary team of healthcare professionals, utilizing advanced therapeutic equipment and methodologies designed to support our patients' recovery journeys. Key Responsibilities

• Deliver compassionate and effective physical therapy services, ensuring a focus on patient-centered care in both outpatient and potential acute care settings.
• Implement individualized treatment plans for each patient as per established protocols, tracking progress and adjusting methods based on patient responses.
• Conduct thorough evaluations, which include reviewing medical histories, performing necessary assessments, and documenting findings to inform treatment strategies.
• Manage discharge planning effectively, ensuring follow-up care is communicated clearly, while adhering to all documentation standards and coding accuracy within our electronic health record system.
• Maintain compliance with all mandated training programs and legal regulations regarding patient safety and health care practices.
• Engage in direct communication with patients, families, physicians, and insurance representatives to ensure a seamless continuum of care and service delivery.
• Participate actively in team meetings, contributing ideas for improving workflows, patient engagement, and therapeutic outcomes.

Patient Population / Client Focus

Our facility caters primarily to orthopedic patients requiring rehabilitation following surgery, injury, or physical trauma. You will work closely with individuals from various demographics, ensuring that personalized care plans align with their rehabilitation goals. Our focus extends to athletes needing specific recovery plans and older adults seeking mobility improvement. This role is pivotal in positively impacting their lives, guiding them through their recovery with empathy and expertise.

Qualifications

• Graduation from an accredited program in Physical Therapy.
• Possession of a Doctorate of Physical Therapy (DPT) degree.
• A valid physical therapy license as required by the state of Virginia.
• Current certification in Basic Life Support (BLS) for Healthcare Providers by the American Heart Association.

Preferred Experience

We are particularly interested in candidates with a minimum of two years’ clinical experience in outpatient rehabilitation settings, demonstrating a history of enhancing patient outcomes through specialized treatment approaches. Familiarity with therapeutic modalities and a commitment to continuous education in evidence-based practices will be an asset.
