Job Summary

High Plains Senior Care Hospice is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) to provide high-quality hospice care to patients in their homes. The RN will perform comprehensive patient assessments, coordinate care with the interdisciplinary team, educate patients and families, and ensure comfort-focused, patient-centered care throughout the hospice journey.

Key Responsibilities
  • Perform initial and ongoing comprehensive patient assessments.

  • Develop, implement, and update individualized plans of care.

  • Provide skilled nursing care and symptom management for hospice patients.

  • Educate patients and their families on disease progression, medications, self-care, and end-of-life care.

  • Coordinate care with physicians and the interdisciplinary hospice team.

  • Monitor patient condition and communicate changes to the physician and care team.

  • Complete accurate and timely clinical documentation and progress notes.

  • Assess caregiver needs and provide appropriate education and support.

  • Make referrals to appropriate community resources and support services as needed.

  • Participate in case conferences and collaborate with the interdisciplinary team.

  • Provide on-call coverage as assigned.

  • Follow all agency policies, state regulations, and hospice standards of care.

Qualifications
  • Current Registered Nurse (RN) license in the State of Texas.

  • Minimum of 1 year of RN experience required.

  • Home Health and/or Hospice experience preferred.

  • Strong clinical assessment and critical thinking skills.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and collaboratively within an interdisciplinary team.

  • Valid driver's license, reliable transportation, and current auto insurance.
