Job Description Summary

The Director of Nursing (DON) is responsible for ensuring that patient care and services is coordinated, delivered, and managed appropriately.

Essential Job Functions/Responsibilities
  • Reviews case referrals. Reviews available patient information related to case, including disciplines required to assist in determining home care needs. Assists scheduling dept with the assigning of appropriate clinicians to case.
  • Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient's clinical diagnosis, medications, procedures and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
  • Holds case conference meetings with agency personnel to facilitate coordination of care.
  • Oversees and meets with clinical staff to disucss performance and provides direction on performance improvment strategies. 
  • Assures quality of patient care delivery and excellent customer service is delivered. 
  • Conducts regular record reviews and communicates findings and recommendations to appropriate agency personnel.
  • May assists in the screening and interviewing process of new agency personnel and make recommendations for employment of individuals. Assists in the orientation of new agency personnel.
  • Assists in the planning, implementation and evaluation of in-service and continuing education programs.
  • Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
  • Complies with accepted professional standards and principles.
  • Supports public relation and community activities that promote the organization's role as an effective member of the health care delivery system.
  • Promotes customer service orientation to all agency personnel.
  • Performs other duties and activities as delegated.

Position Qualifications
  • Registered Nurse with current licensure to practice professional nursing in the State.
  • Graduate of accredited school of nursing. 
  • Prefer management or supervisory experience in a health care setting, preferably home care, but would consider a seasoned home care field RN looking for an opportunity to move into a clinical leadership role.
  • Complies with accepted professional standards and practice.
  • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
  • Complies and maintains current CPR certification.
  • Demonstrates excellent observation, verbal and written communication skills.
