Human Touch Home Health is seeking compassionate and dependable Registered Nurses (RNs) for Full-Time and PRN opportunities with our Blacksburg team.

You'll provide skilled nursing care to patients throughout Blacksburg, Christiansburg, Radford, Pulaski, Floyd, and surrounding New River Valley communities.

What You'll Do
  • Complete skilled nursing assessments and home visits
  • Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
  • Develop and update individualized plans of care
  • Provide wound care, medication management, and disease education
  • Monitor patient conditions and communicate significant changes
  • Educate patients and caregivers
  • Coordinate care with physicians, therapists, and other team members
  • Complete accurate and timely documentation using WellSky
  • Help patients safely manage their health at home
Qualifications
  • Active Virginia Registered Nurse (RN) License Required
  • Graduate of an accredited nursing program
  • Current CPR/BLS Certification
  • Valid Driver's License and reliable transportation
  • Strong clinical assessment and communication skills
  • Ability to work independently in patients' homes
  • Home Health and OASIS experience preferred
  • WellSky experience is a plus
