Position Summary
The RN Home Care Assessment Nurse conducts comprehensive in-home evaluations for new and existing clients to determine eligibility, level of care, safety risks, and service needs. This nurse plays a critical role in ensuring high-quality, compliant, and patient-centered care.
Key Responsibilities
- Conduct initial and follow-up in-home nursing assessments
- Evaluate physical, cognitive, and psychosocial status
- Identify safety risks, fall risks, and home environment concerns
- Verify diagnoses, medications, and care needs
- Determine level of care and service eligibility
- Document assessments clearly and accurately in the medical record
- Communicate findings to care coordinators and clinical leadership
- Support regulatory compliance with CMS, state, and agency policies
Qualifications
- Active Registered Nurse (RN) license in [State]
- Minimum 1 year of clinical experience (home health, hospice, or case management preferred)
