Position Overview

At our organization, our team members transform lives and instill hope through genuine care. As a Hospice Physical Therapist, you will plan, organize, and deliver therapy services aimed at maintaining comfort, function, and quality of life—not active rehabilitation. You will guide patients and caregivers through supportive techniques, safe mobility, and meaningful education to promote dignity and ease during the end-of-life journey. Every interaction reflects our core values: trustworthy, capable, compassionate, proactive, and dedicated.

Key Responsibilities

Patient Care

  • Deliver physical therapy services in accordance with the physician’s care plan and state regulations.
  • Assess and evaluate therapeutic, rehabilitative, and functional status; contribute to the overall care plan.
  • Evaluate the home environment to ensure patient safety and comfort.
  • Assess muscle strength, mobility, gait, range of motion, and transfer abilities, focusing on comfort rather than rehabilitation.
  • Administer physical therapy treatments and educate patients/families on the use and maintenance of therapeutic devices.
  • Identify and prioritize therapy needs, reporting patient responses or changes in condition to the physician.
  • Initiate therapy programs and instruct family/caregivers and interdisciplinary team members on therapy techniques and care objectives.
  • Train patients in the use of prosthetic devices when applicable.
  • Recognize the need for additional home health services and make referrals as necessary.
  • Provide consultation to patients, families, and caregivers.

Communication & Documentation

  • Prepare and submit clinical notes, progress reports, and discharge summaries in line with organizational policies.
  • Engage in patient discharge planning to facilitate smooth transitions in care.
  • Communicate effectively with physicians, caregivers, and interdisciplinary team members.
  • Participate in interdisciplinary group (IDG) meetings to support coordinated hospice care.

Additional Duties

  • Provide in-service training for nursing and organizational staff as needed.
  • Engage in peer consultation and Quality Assurance Performance Improvement (QAPI) activities.
  • Supervise Physical Therapy Assistants (PTAs) in accordance with policy and state regulations.
  • When therapy is the sole skilled service, instruct, supervise, and evaluate home health aide care biweekly.

Qualifications

  • Bachelor’s or master’s degree in physical therapy from an accredited institution.
  • Licensed to practice as a physical therapist in the relevant state.
  • Preferred two years of relevant experience; experience in home health or community settings is advantageous.
  • Strong verbal, written, and organizational abilities.
  • Current CPR certification.
  • Valid driver’s license with an insured vehicle that meets organizational standards.
