Job description

LIFESPAN Services is hiring a compassionate, hardworking, friendly, and reliable Life Coach to be motivational, inspiring, positive, exciting, and action-driven while working with an assigned caseload of individuals in a community setting.

Schedule:

Monday through Thursday 8:00 AM to 2:00 PM; Friday 8:00 AM to 1:00 PM or Monday through Thursday 9:00 AM to – 3:00 PM; Friday 9:00 AM to 2:00 PM

What you’ll do:

  • Teach skills to promote independence.
  • Support inclusion and integration.
  • Mentor individuals to help build social skills and relationships.
  • Provide guidance and support in daily activities.
  • Assess and develop tools to help the gap between where they are in their personal lives and where they would like to be as indicated in the Person-Centered Plan.
  • Raise awareness of appropriate and inappropriate actions and encourage individuals to take responsibility for their lives. E.g. achieving goals, finding balance in life, relationship enhancement, health and wellness and personal growth.
  • Empower and promote independence by exploring opportunities for individuals to make healthy choices in their lives.
  • Assist individuals with rising to their own particular challenge and go further than they ever thought possible.
  • Participate in and provide input into individual’s PCP (Person Centered Plan).
  • Ensure that goals and plans are implemented.
  • Oversee and direct the activities of individuals while performing job duties and/or volunteer community activities.
  • Assign, review and plan work for individuals, ensuring that they are placed in the most appropriate and least restrictive job for their individual needs and interest.
  • Work as liaison and utilize marketing strategies between the community and individuals to ensure needs are met.
  • Provide support for individuals in a variety of ways such as assessment, job development, training, technical assistance, monitoring safety conditions, providing transportation and devising weekly schedules.
  • Establish and maintain good relationships with various individuals and agencies including area supervisors, referring agencies, parents/guardians, care coordinators, employers, etc.
  • Encourage the use of natural supports at the worksite and/or volunteer community activities.
  • Assist with participation in leisure/recreational activities.
  • Use electronic health records to document activities and significant events.
  • Submit documentation and payroll data according to scheduled deadlines.
  • Complete initial and ongoing training, and participate in staff development activities.
  • Provide transportation to and from designated sites.

What you need:

  • Must be at least 18 years of age
  • Pass criminal record and healthcare registry check
  • Pass driving record check if providing transportation
  • High school graduate or equivalent (GED)
  • Commitment to achieving competency in required knowledge, skills, and abilities
  • Note: Current First Aid/CPR blended or classroom certification is required. If you do not currently have certification from an approved provider, our American Red Cross certified instructors will provide you the training.
  • Ability to lift up to 40 pounds independently (per policy)
