Job description

Job Summary  
We are seeking a full-time Speech Language Pathologist/CF for a Skilled Nursing Facility. The SLP/CF will work closely with the adult population with complex communication needs. Great opportunity to join a collaborative rehab team in a supportive SNF environment. Mentored experience transitioning from clinical fellow to independent professional.   

Job Responsibilities  

  • Conduct comprehensive assessments and provide evidence-based treatment of speech, language, cognitive and swallowing abilities.  

  • Develop individualized treatment plans tailored to each patient’s needs.  

  • Collaborate with families, educators, and healthcare professionals to support patient progress.  

  • Complete accurate and timely documentation including daily notes, progress reports, evaluations, and discharge summaries.  

  • Stay current with advancements in speech therapy practices and medical terminology.  

  • Ensure compliance with facility protocols and state/federal regulations.  

Experience & Qualifications:  

  • Master’s degree in Speech Language Pathology  

  • Valid NY SLP License. CFs are welcome to apply!  

  • Strong communication and organizational skills contributing to a collaborative environment   

  • Knowledge of dysphagia and cognitive rehabilitation preferred  

  • Experience in a skilled nursing facility preferred  

  • Experience with Net Health/Rehab Optima is a plus
