We are seeking a full-time Social Services Director / long-term care Social Worker to provide assessment, advocacy, and discharge planning for the psychosocial needs of patients of all ages. This role requires expertise in crisis intervention, mandated reporting, and care coordination for patients with psychosocial risk factors. Responsible for education and advocacy for patients, families, and significant others.

The Social Services Director coordinates the psychosocial needs of residents and families and works closely with the Nursing Department to support resident care in a long-term care / healthcare facility.

Social Services Director Requirements:

  • Bachelorโ€™s Degree in Social Services

  • CSW (Certified Services Worker) certificationโ€”State of Missouri

  • Knowledge of community resources

  • Experience in healthcare social work

  • Ability to prioritize multiple tasks

  • Strong interpersonal and communication skills

  • Professional demeanor with patients, families, interdisciplinary teams, and outside agencies

  • 2โ€“5 years of acute care or long-term care experience preferred

  • MDS experience preferred

  • Long-term care social work experience a plus
